    **Description**

    **Role Overview**

    The Team Leader ensures that medical, emotional and safety needs are met for individuals; advocates for individual rights and needs; facilitates habilitation; and maintains finances and funding for individuals. The Team Leader also manages, trains and develops direct care staff; works in cooperation with and responds to inquiries from individuals, families, and others, internally and externally; and performs administrative tasks. Duties and responsibilities for this position are carried out in accordance with the philosophy and mission of New Hope of Indiana.

    **Primary Role Responsibilities**

    A. Ensures individual's emotional, medical, and safety needs are met.

    B. Advocates for individual's services and rights.

    C. Facilitates habilitation by assisting with writing and achieving goals; mentoring and encouraging progress.

    D. Coordinates the care of individuals with consultants and others, both internal and external.

    E. Manages and safeguards finances according to each individual's competency.

    F. Maintains Medicaid and Social Security funding.

    G. Develops and maintains relationships with individuals, families, staff and others, internal and external.

    H. Participates in compensated on-call rotation.

    I. Maintains appropriate staffing levels.

    J. In collaboration with HR and management, interviews & selects, evaluates, and manages the disciplinary process for subordinate associates.

    K. Trains, develops and coaches staff.

    L. May occasionally direct activities of volunteers.

    M. Participates in training and other staff meetings.

    N. Performs administrative duties including documentation, billing, payroll, and budgeting.

    O. Models Corporate Responsibility ethical business behaviors.

    P. Serves as a "community connector", attempting to connect each individual with as many naturally occurring supports within the community as possible (i.e. neighbors, friends, coworkers, family members, etc.)

    Q. Ensures adherence to New Hope policies and procedures.

    R. Incorporates Core Values and Mission of New Hope in the work environment.

    S. Performs other duties as may be assigned.

    **Requirements**

    **Required Experience and Qualifications**

    ***Required:***

    High School Diploma, GED or equivalent

    One year of experience working with individuals with disabilities.

    **Preferred:**

    Bachelor's Degree in a Human Services related field

    Supervisory experience

    **Eligibility, Special Training, Licensure or Credentialing**

    Must possess a valid Indiana Operators License and be willing to use personal vehicle as needed. Must meet New Hope driving record guidelines and have proof of current automobile insurance.

    **Work Schedule and Location**

    This position is an exempt level position. Must be willing to work flexible hours, including evenings, nights and weekends.
