- Develop relationships and collaborate with primary care tea to conduct social services assessments to determine the appropriate needs for each member.
- Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.
- Navigate managed care plans for community services and programs.
- Experience working with patients in need of social services.
- Experience with assessments and planning for social services.
- Working knowledge of different diagnosis of common health conditions.
- Fully licensed LMSW in the desired State of practice.
- Master's degree in Social Work.
- A problem-solving orientation and a flexible and positive attitude
- Mission driven and motivated to join an organization that will transform the way we deliver accessible client excellent care to seniors.
- Proficient PC skills.
- Fluency in Spanish or other language spoken by people in the communities we service.
