    General Summary


    The Home Health RN acts as a primary care manager for a caseload of patients to assess, plan, implement, and evaluate the plan of care, under the direction of and in cooperation with the patient's physician for a group of patients in an assigned territory within Ashtabula or Eastern Lake and Eastern Geauga Counties.


    • Mileage and Phone Stipend.

    • Flexible scheduling opportunities to meet individual family needs. Mon through Friday 8- or 10-hour, Full Time or Part Time options. Weekender/ON CALL positions also available.
    • Start and end your day from your own home. Attend most meetings remotely.
    Minimum Qualifications

    Education, Knowledge, Skills and Abilities


    • Degree in nursing required, BSN preferred.
    • Required professional skills include data entry into and retrieval from a managed care information system, and ability to effectively interface with physicians.
    Required Length and Type of Experience


    • One year nursing experience preferred
    • New graduates welcome Individualized Orientation Program with Mentor program for new graduates
    Required Licensure or Certification

    Current State of Ohio nursing license.
