General Summary
The Home Health RN acts as a primary care manager for a caseload of patients to assess, plan, implement, and evaluate the plan of care, under the direction of and in cooperation with the patient's physician for a group of patients in an assigned territory within Ashtabula or Eastern Lake and Eastern Geauga Counties.
