    Description

    Community Health Outreach Worker at Guilford County Formerly Incarcerated Transition Program

    The main role of this position is to assist participants in connecting with health information and services, while aiding in overcoming barriers to re-engaging in society post-incarceration. Working closely with community agencies and Health and Human Services organizations, you will play a crucial role in addressing the social determinants of health.

    Special Requirements:
    • Proven history of successful reentry into the community following at least three years post-release from incarceration.
    • Valid North Carolina driver's license with a favorable three-year driving record.
    Responsibilities:
    • Connect residents with community resources providing educational programs and support services.
    • Educate participants on medication access options and services.
    • Assist in building provider-patient care relationships.
    • Help participants navigate health coverage resources or apply for assistance if uninsured.
    • Service locations include clinics, home visits, transition centers, or other community settings.
    • Engage in Transition Care clinic network conferences, trainings, and meetings.
    • Participate in NC FIT Program phone calls, webinars, and meetings.
    • Attend required trainings to effectively engage the population.
    • Represent the organization at various local community meetings.
    Experience Requirements:
    • Strong written and oral communication skills.
    • Ability to work independently and collaboratively.
    • Basic computer and word processing proficiency.
    • Previous experience in peer support work is a plus.
    • Flexibility for early morning, evening, and weekend assignments.
    • Minimum one year of relevant experience in healthcare, social services, or similar fields.
    Education:
    • High school diploma or GED equivalent preferred.
    • Prior experience in peer support work is beneficial.
