Job Summary


This position provides a variety of community-based employment services to individuals with disabilities seeking to become employed, advance in their work or re-enter employment.

Incumbent must be an initiative-taker with effective communication, time management and problem-solving skills. Incumbent is expected to manage personal caseload and also provide support to team members.

Primary Duties

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. These duties and responsibilities will be rated on the Annual Performance Review.- Develop, implement, and document Individualized Service Plans for individuals seeking Employment Services as identified in current contracts, including but not limited to career exploration, skills training, pre-employment training, situational assessments, job development, job coaching and long-term supports for both competitive and supported employment).- Build and maintain professional and productive relationships with participants, members of participants support network and funding/referral sources, providing on-going communication soliciting input, requesting satisfaction surveys and providing thoroughly documented reports.- Develop relationships with current and potential employers resulting in knowledge of current employment, assessment and training opportunities for participants. Request input and feedback from employers.- Update participant plans as needed to ensure they reflect current goals.

Update case files at least monthly and update electronic databases as change occurs.- Maintain participant files according to policies and procedures.

Ensure files consistently meet standards for regulatory entities. Ensure information is updated and accurate in electronic databases.


Additional Responsibilities- Work in collaboration with Employment Services team members.- Implement employment service policies and procedures.- Demonstrate creativity and flexibility balancing the interests of individual with those of referral sources and employers.- Continuously maintain an awareness of local labor market needs.- Practice self-driven learning seeking new information and resources to support goals of individuals served.- Provide Social Security benefits assistance.- Serve as an advocate for individual served at all times.- Assist in Marketing activities; submitting stories for social media and newsletters, creating materials, participating in community activities and/or provider events coordinated by Referral Agencies.- Provide travel training, identify transportation for individuals prior to employment.- Perform other responsibilities as assigned.


Qualifications:
Education, Experience, and Certification(s)

  • Bachelor's Degree. Preferred degree in Special Education, Psychology, related social service and/or equivalent work experience is required.
  • Minimum of two year's work experience required.
  • Valid driver's license and/or access to reliable transportation to perform workrelated travel required. Eligible drivers must have a good motor vehicle record (MVR).


Knowledge, Skills, and Abilities- Demonstrate effective verbal and written communication skills.- Proficiency in Microsoft Office skills or the ability to learn.- Ability to learn ServiceSource specific programs and software, and software required by contracting agencies.- Ability to obtain CESP certification within 24 months of employment.- Must be a self-starter and able to develop service plans for individuals with a variety of disabilities and required accommodations.- Demonstrate respect for a diverse population, including but not limited to race, ethnicity, religion, socio-economic status, and gender identification.- Able to work independently and as a member of team.- Ability to work evenings, weekends and holidays as needed.
