    Description

    Salary Range:
    $ $ /hour. Actual compensation may vary based on geographic location, work experience, skill level, and education.

    Under the supervision of the RN Care Manager, the Care Manager Assistant carries out key responsibilities in providing Care Management services for Hoag Hospital and its affiliates (referred to as the 'Organization').

    The Care Manager Assistant executes patient discharge plans, aids in coordinating clinical utilization review, and communicates with third party payers in support of the nursing and social work team within the Care Management department.

    Additionally, the Care Manager Assistant engages with representatives from insurance companies, health maintenance organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.

    The Care Manager Assistant demonstrates a work approach that aligns with the mission, vision, and values of the Care Management team and the Organization, while actively participating in the ongoing quality improvement initiatives.

    Maintaining proficiency in departmental standard operating procedures, which includes the use of specialized tools, adherence to Quality Control standards, and performing preventive maintenance tasks.

    Qualifications - Education, Training and Experience:
    • High-School graduate or equivalent.
    • Minimum of one-year experience in administrative assistant or clerical roles.
    Skills or Other Qualifications:
    • Self-motivated individual with strong task prioritization skills, ability to manage multiple tasks and meet deadlines, exceptional attention to detail, and commitment to delivering work of high quality.
    • Effective team player with outstanding verbal, written, and presentation communication abilities.
    • Demonstrated capacity to interact with individuals at all levels (such as physicians, staff within the Organization, insurance entities, patients, and families).
    • Familiarity with Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint, as well as experience with healthcare database systems.
    • Basic understanding of medical procedures and terminology.
    • Desired:
    • Candidates proficient in multiple languages.
