- Prepare, file, and track electronic paperwork submissions
- Oversee the comprehensive MDS assessment process, ensuring accurate and complete documentation.
- Collaborate with nursing and therapy teams to develop individualized care plans based on MDS assessments.
- Train and educate staff on MDS assessment processes and documentation.
- Ensures that the facility adheres to all regulatory requirements and current standards of practice in relation to the MDS care planning process
- Attend Care plan assessment meetings
- Completing all required documentation for medical records, looking for trends and areas for improvement.
- Collaboratively works with the resident, family, and interdisciplinary team.
- Review clinical records for accuracy and compliance with MDS coding.
- Active RN license from the State of NJ - a must
- Experience in long-term care and at least 2-year job-related experience in the MDS process.
- Supervisory experience
- In-depth understanding of Federal and State regulations related to MDS
- MDS 3.0 expertise, very familiar with PDPM
- Strong Experience with Medicaid - CMI scoring
- Must have strong familiarity with EMS and MDS software.
- Must be efficient, organized, detail-oriented, and flexible.
- Self-directed but team-oriented
