Affiliated with Danbury Hospital, Nuvance Health Medical Group is seeking a full-time Geriatric Nurse Practitioner (APRN) to work independently to provide primary care for 2 Skilled Nursing Facilities, River Glen and The Lutheran Home, in Southbury, CT.
This is a position to support our efforts to provide Primary Care services to Skilled Nursing Facilities.River Glen is part of Southbury's South Britain Historic District, which is located along the picturesque Pomperaug River. The Lutheran Home is part of Southbury's Main Street Historic District, displaying many historic structures and landscapes.
Both skilled nursing facilities provide expert care to patients in long-term care and short-term rehab and focus on personalizing an individual care plan for each patient to meet their specific needs.
A three-month mentorship program is part of your initial training experience.Job Responsibilities:
The APRN provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses.
