Responsibilities:

Patient Assessment and Evaluation: Conduct comprehensive health assessments of patients, including obtaining medical histories, performing physical examinations, and ordering and interpreting diagnostic tests.
Diagnosis and Treatment: Diagnose and treat acute and chronic illnesses, injuries, and conditions across the lifespan, utilizing evidence-based practice guidelines and clinical expertise.
Health Promotion and Disease Prevention: Develop and implement individualized care plans focused on health promotion, disease prevention, and management of risk factors, incorporating patient education and counseling.
Collaborative Care: Collaborate with physicians, advanced practice providers, nurses, allied health professionals, and other members of the healthcare team to coordinate patient care, facilitate referrals, and ensure continuity of care.
Patient Advocacy: Advocate for patients' rights, preferences, and access to quality healthcare services, while promoting shared decision-making and informed consent in the treatment process.
Documentation and Record Keeping: Maintain accurate and timely documentation of patient encounters, assessments, interventions, and care plans in electronic health records (EHR) systems, ensuring compliance with regulatory requirements and professional standards.
Quality Improvement: Participate in quality improvement initiatives, performance evaluations, and peer reviews to enhance clinical practice, patient outcomes, and satisfaction with care delivery.

Qualifications:

Master's or Doctoral degree in Nursing from an accredited program.
Current, unrestricted license to practice as a Nurse Practitioner (NP) in the state of New York.
Clinical experience as a Nurse Practitioner preferred, but new graduates with relevant clinical rotations may be considered.
