    We are seeking a skilled Licensed Vocational Nurse (LVN) to join our Palliative Care program in Cerritos. This role is not for hospice but specifically focuses on providing coordinated care and support to our patients and their families. This is an excellent opportunity for someone with strong administrative skills and a passion for providing exceptional patient service.

    Responsibilities:

    • Coordinate admissions and handle insurance matters for our Palliative Care program.
    • Schedule visits for Nurse Practitioners and maintain detailed records of patient care.
    • Communicate effectively with medical groups and provide exceptional customer service.
    • Assist in the scheduling of patients across various disciplines and ensure coordination of care.
    • Maintain accurate patient care data in our systems and prepare reports for providers and referral sources.
    • Handle phone inquiries and triage when necessary.

    Requirements:

    • Current LVN licensure in the state of California.
    • Associate's degree in Nursing from an accredited program.
    • Minimum of two years' experience in coordinating admissions, insurance verification, or healthcare operations, preferably in a facility or hospice setting.
    • Strong communication, negotiation, and public relations skills.
    • Ability to work autonomously with excellent organization and flexibility.
