*You must be a local resident. No out-of-state applicants.
We're seeking a person to assist our admission coordinator with discharge planning experienced with the senior population. Familiar with Medi-Cal housing programs. Experience in an Assisted Living Facility working with the elderly. You have networked and developed relationships and sales strategies with hospitals and communities to build on potential residents. You're well-versed with the Assisted Living Waiver, Regional Centers, and PACE programs.
A. DUTIES AND RESPONSIBILITIES
- Overall Care Management of participants of the Assisted Living Waiver (ALW) & Regional center programs.
- Familiarity with the Assisted Living Waiver (ALW), Regional Center, and PACE Programs.
- Assist residents with Medicare & Medi-Cal benefits. Familiar with Social Security and Medical resources.
- Hospitals and discharge planners to market the facility.
Maintains effective communication with residents, family, Service coordinator, Social worker, Clinical Supervisor, and other health care team members in regard to residents' needs and progress.
- Develops the Appraisal of Needs/Service plan for program participants.
- Works with admission with MOVE IN's.
- Ensure that all leads, tours, reservations, repeat tours, phone calls, "move-ins,'' and ''move-outs" referrals are contacted and followed upon.
- Ensures enrollment status remains current for ALW and Sales clients. Ensures that brochure requests are mailed and followed upon.
- Ensures that a supply of brochures are on hand for walk-ins.
- Ensure that all leads and all communications are documented in the CRM platform; Casebook.
- Makes rounds each morning to ensure that our team has faxes.
- Obtain a weekly Update for Case Manager Availability, and update Programs about vacancies.
- Educate discharge planners and social workers from hospitals and SNFs on ALW program.
- Submit a Weekly Report of Projected and Actual Plan.
- Build community awareness by contacting community resources, centers, and groups.
- Coordinates with the Activities Dept. to have activities that advance community outreach.
- Maintains liaison with families and residents.
- Complete Assessments (functional capability, head-to-toe) for prospective and current residents.
Attend the following meetings:
a. Monthly budget meetings with the Account Receivables department. b. Case Management meetings. c. Quarterly Regional center client meetings.
d. Monthly ALW social worker meetings.
Maintain the following reports:
a. Track program participants that are in SNF's/hospitals. Follow residents admitted to
SNF/Hospital & keep up-to-date on progress/dc plans b. Track wellness program c. Track Medi-Cal Reinstatement. d. Track census e. Track ALW Assessment Deadlines & Tier Levels f. Makes rounds each morning to ensure that the facilities are ready for tours
B. MINIMUM QUALIFICATIONS
- Develop, and establish close relationships by networking with hospitals and discharge planners for prospective new residents.
- Sales experience working in an assisted living facility environment.
- Presents a professional image to prospects through dress, behavior, and speech.
- Experience working with older adults and seniors.
- Possesses Common sense.
- Familiarity with the Assisted Living Waiver Program (ALW) and Regional Center Programs
- Ability to work well with our demographic: seniors and adults with developmental, cognitive, or memory impairments.
- Current training in First Aid and CPR.
- Good written/verbal communication skills
- Valid driver's license
- Familiar with Social Security and Medi-Cal resources.
- Professional appearance and positive attitude
1. experience within an Assisted Living Facility environment. A highly energetic and organized individual who has a positive, "take ownership" attitude.
- Highly competent to work effectively with families/prospects seeking non-medical care.
- Have sufficient background knowledge and expertise in assessment, evaluation, and skills to meet the needs of his/her residents and to contribute to Quality Management review and evaluation

Pay:
$ $28.00 per hour
