- Current licensure as a Nurse Practitioner in the state of New York.
- Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
- Minimum of 3600 hours of clinical experience with 1 year of clinical experience in a primary care or longitudinal care setting.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and patient education abilities.
- Proficiency in electronic health record (EHR) systems.
- HEDIS/STAR measures experience
- Homebased visit experience
- Hierarchical Condition Category (HCC) documentation experience
- Ability to complete physical requirements of the job including navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment.

Position Overview: We are seeking a dedicated Nurse Practitioner to join our care team in New York. Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
Documentation: Maintain accurate and up-to-date electronic health records (EHR) to track patient progress, treatment plans, and interventions.
Follow-Up: Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
