Job Title:
Case Manager, Home Health, Hospice
Duration: 13 weeks with possible Extension

Location:
Littleton, Berlin, NH

Days: 40hours/Days; 5*8 days per week

Tentative Start Date: 05/20/2024


Requirements:


  • Minimum of 2 years CURRENT
    Home Health experience in Case Management and Hospice Experience:

  • RN will be working in the Whitefield/Berlin, NH area:
  • RNs will be providing Home Health & Hospice Case Management services to patients of all ages (infant to elderly)
  • Netsmart/Allscripts EMR experience strongly preferred
  • Mileage paid at current approved Federal IRS Rate (Mileage paid from office each morning)
