- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es)
- Regularly re-evaluates patient nursing needs
- Initiates the plan of care and makes necessary revisions as patient status and needs change
- Uses health assessment data to determine nursing diagnosis
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician
- Counsels the patient and family in meeting nursing and related needs
- Provides health care instructions to the patient as appropriate per assessment and plan
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient
- Prepare and maintain clinical documentation in a timely and accurate manner in compliance with company policy
- Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required
- Communicates with community health related persons to coordinate the care plan
- Participates in on-call duties as defined by the on-call policy
- Ensures that arrangements for equipment and other necessary items and services are available
- Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
- Ensure individual continuing education and personal/professional development responsibilities are satisfied in a timely manner
- Maintain required licensures and certifications; provide the company with the most up-to-date copies of such documents in a timely manner
- Demonstrate working knowledge of Medicare reimbursement criteria, current documentation needs, ordered visit frequencies, dates of patient episodes, and dates recertification would be needed, if appropriate, for all patients under his/her care and supervision
- Maintain standards of practice and documentation that are consistent with Federal, State, Local, Medicare, and accreditation regulations and standards
- Demonstrate flexibility while performing job duties to ensure the highest quality of service is provided
- Utilize critical thinking skills while performing job duties; analyze work process and make suggestions for improvement
- Maintain a thorough understanding and consistently adhere to all applicable department and company policies and procedures
- Conduct job duties in a way which will reduce and avoid potential safety hazards
- Demonstrate initiative and drive to promote personal and company success
- Perform other duties as assigned by your supervisor or management team member
- Licensed in the state as a registered nurse by the Board of Registration in Nursing
- Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred
- Possesses and maintains current CPR certification
- Complies with accepted professional standards and practice
- Maintains current continuing education as required by state licensure
