    Description
    Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. Nursing Case Manager


    Daily tasks include but are not limited to coordination of client discharges, intake assessments of new clients, auditing of in home medications and medication administration records, management of new medication orders and training of staff for new diagnoses.

    The ideal candidate must possess a strong understanding of basic nursing concepts including pharmacology. A general understanding of computers such as email, excel and Microsoft word is beneficial. Associate's Degree in Nursing or related field, or a graduate of an accredited LPN program.

    • One year of experience working with population served in a nursing capacity.
    • Current driver's license, car registration, and auto insurance.
    • Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training.
    • Ability to effectively communicate with staff, individuals served, and families.
    • You will make a difference every day and help to provide quality of life enhancing services to the individuals we serve.
    We offer full-time Monday-Friday with flexible start times, and no on call responsibilities.
