    Description

    Position Summary: Case Managers / Counselors are responsible for providing individual, group, and family counseling services, along with completing necessary documentation for the clinical record. This includes developing Individual Treatment/Service Plans, conducting psychosocial assessments, maintaining daily notes, and participating in interdisciplinary treatment team meetings. They also handle crisis intervention, ensure client safety, and design counseling curriculums.
    Essential Job Functions:

    • Assessing resource needs of patients.
    • Conducting client screening, assessment, and treatment planning based on individual strengths and preferences.
    • Maintaining a secure delivery system for client confidentiality and safety.
    • Interviewing potential clients, providing crisis intervention, and offering direct counseling services.
    • Monitoring client behavior and creating Individual Treatment/Service Plans.
    • Reporting incidents to the appropriate authorities and educating patients on community resources.
    • Assisting patients with support system development and advocacy for needed services.
    • Participating in clinical decision-making and documentation.
    • Providing input on patient treatment needs and disposition.

    Qualifications:

    • Collaborative and professional demeanor.
    • Ability to work effectively in a team environment.
    • Good judgement, discretion, and computer skills.
    • Excellent verbal and written communication abilities.
    • Willingness to engage in new projects and contribute to service improvement.

    Education:

    • Bachelor's Degree in social work, psychology, education, counseling, or related field preferred.
    • CAP certification preferred.
