    Description
    We are seeking a passionate RN Case Manager to join Patient's Choice Home Health Care a subsidiary of Nomi Health in Miami, Florida. You will report to the DON. You will be responsible for managing all aspects of the patient care process including assigning and overseeing home care personnel. You will establish and maintain positive relationships with patients, referral sources, and respond to patient questions and concerns. We provide skilled nursing, physical/occupational therapy, personal care and other home health services in Miami Dade and Monroe County, with the highest regard for quality of care and respect for our patients.

    How you will make an impact
    • Directs all daily patient care/assignment; works cooperatively with field
    • Ensure all patient needs are met; assists with community
    • Transfers/referrals to other providers, follows up to ensure completion
    • Documents all activities in client's file. May order DME and supplies
    • Directs the implementation of improved work methods and procedures to
    • Ensure patients are admitted/recertified discharges in accordance with policy
    • Establishes and maintains positive working relationships with current and potential referral sources
    • Performs case conferences regarding patient progress, needs, and all pertinent concerns related to patient care
    • Builds and monitors community and patient perceptions of Agency as a high-quality services provider
    • Ensures supervisory visits are performed in accordance with guidelines
    • Assumes responsibility for the Agency in the absences of the Administrator and DON if requested
    • Upholds patient and Agency confidentiality
    • Assist with other duties as requested
    What we are looking for
    • High school diploma or equivalent
    • Licensed RN (Registered Nurse)
    • 1 + years' experience in healthcare setting, preferably in home health
    • Previous experience in case management or a related field
    • Familiarity with medical terminology, social service systems, and healthcare resources
    • Excellent interpersonal and communication skills to build rapport with clients and collaborate with interdisciplinary teams
    • Ability to empathize with clients' challenges and provide support with a compassionate approach
    • Strong problem-solving skills to assess clients' needs and develop effective care plans
    • Ability to manage multiple cases, deadlines, and paperwork efficiently
