    Job Summary

    This position is responsible for the facilitation of all aspects of our Staying Strong Aftercare Program at Rising Strong, in addition to helping support families in engaging in Foundational Community Supports (FCS) for on-going formal (housing & employment) supports both before and after families exit the program. This position includes program administration, data tracking and organizing program events & public-facing education to community partners. Ability to flux schedule around weekend and after hours events required.

    Job Duties/Responsibilities

    1. Assist RS families in developing a clear list of the goals they wish to achieve upon moving out of Rising Strong.
    2. Create a case management file (either hard copy or via Salesforce) for each RS alumni, documenting case management services received, progress toward goals, and all applicable ROI's and program compliance documentation.
    3. Act as a liaison, as needed, with other community agencies to assist alumni in meeting their self-identified goals.
    4. Organize and host bi-monthly alumni events, yearly Resource Fairs and other alumni-specific activities in partnership with WISe Program Manager.
      1. As applicable: coordinate transportation to and from events and all aspects of the event workflow.
    5. Support & attend scheduled graduation & celebration events.
    6. Facilitate survey administration/data collection for all alumni enrolled in the program to ensure qualitative data is collected & captured, aligning with program goals.
      1. Provide monthly reports, documenting aftercare activities for funding sources.
      2. Directly oversee all data tracking elements.
    7. Provide feedback, as needed, to assist alumni with problem-solving, conflict resolution, parenting and interpersonal concerns and attainment of goals.
    8. Oversee training of alumni in budgeting, rental rights and responsibilities and community awareness skills.
    9. Provide access to healthcare resources to maintain mental and physical healthcare for alumni families.
    10. Assist families with maintaining housing/voucher requirements and accessing employment resources.
    11. Conduct home visits for on-going support, where applicable.
    12. Integrate Catholic Charities-wide efforts of addressing social determinants of health in housing, ensuring all services are trauma informed, and implementing strategies of ensuring services and hiring practices are addressing race, equity and inclusion disparities.
    13. Collaborate in the design and implementation of community awareness and marketing plan for the program.
    14. Serve as primary ongoing point of contact for key partners who have questions about our Staying Strong program.
    15. Advocate for systemic change that improves the lives of those served at RS; represent Rising Strong in the community.
    16. Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
    17. Regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by the Managing Director.
      1. This includes checking and responding to email and text multiple times throughout the day to ensure that all touchpoints of communication are triaged for urgency & responded to in a timely manner/ no later than 24 business hours.
    18. As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
    19. Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.
    20. Perform related functions, including advocacy, to support the mission of Catholic Charities.

    Job Qualifications

    To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.

    Education/Experience: to perform this job successfully, an individual must have a bachelor's degree in a related field or an equivalent combination of education and experience. Experience in program administration preferred. Three years case management experience required. Knowledge of community resources required; experience working with issues of homelessness, child welfare, poverty, and violence against marginalized groups preferred.

    Certificates/Licenses: Must possess a valid Driver License and ability to drive for work use. Successfully pass background check applicable to position.

    • Requires respirator medical evaluation questionnaire, fit testing, training, and use of respiratory protection as required by the WA Industrial Safety & Act standards. Refer to WAC to learn more.

    Physical Abilities: To perform this job successfully, an individual must be able to:

    Workplace Environmental Conditions

    While performing the essential duties/responsibilities of this job, the employee will be:

    • Client Population may include persons involved in gangs, with substance abuse concerns, homelessness and/or tendencies towards violent behavior.
    • Noise Conditions: exposed during a shift to constant or intermittent sounds at moderate to loud levels of sound dependent upon the activities of the Shelter.
    • Exposure to Trauma: Employees of Catholic Charities Eastern Washington provide direct services to vulnerable community members including those who have behavioral health challenges and those who have been exposed to trauma.
    • Potential for exposure to illicit drugs and air-borne contaminants (proper PPE provided).
