    Program Summary

    Rising Strong serves families who are at risk for separation due to parental substance misuse by providing safe housing, substance misuse treatment, case management, and other supportive wraparound services. By serving whole families, our aim is to reduce trauma in children, decrease the rates of foster care placements, and remove barriers that traditionally prevent parents from succeeding in substance abuse treatment. We walk alongside and support vulnerable families as they seek to change their stories.

    Job Summary

    The Care Coordinator is a member and facilitator of a cross-system team to serve families and their children with challenging behaviors through Wraparound Intensive Services (WISe). This position works with the child and family to develop and implement individualized care plans that work toward the family's long-term goals. Services are provided where and when convenient for the family, including home and community-based locations for alumni families. The Care Coordinator will assist as a member of the team to implement positive behavioral support in the home. Care Coordinators must be skilled in facilitation, managing conflict, engaging cross-system team members and communicating with diverse stakeholders and natural supports.

    Job Duties/Responsibilities

    1. Assist in completing family and behavioral health assessments with child and parent, including CANS (child adolescent strengths and needs).
    2. Participate in decision making for high-risk families, contributing to the development and implementation of individualized treatment plans (Cross-System Care Plan, Crisis and Safety Plan, and Individualized Service Plan) that outline cross-system care, strengths, goals, and specific action steps.
    3. As part of the clinical team, assess the family's need for community resources to ensure safe and recovery-oriented, trauma-informed treatment. Provide referrals and planning for resources.
    4. Provide and create a supportive environment for parents and caregivers by encouraging hope in, and control over, their own lives as well as their children's lives.
    5. Promote self-determination, empowerment, personal responsibility for recovery and wellness, and assist parent/caregivers to advocate for themselves and their children.
    6. Collaborate with the treatment team to promote a team culture in which decision-making in treatment and the cross-system's plan ensures the child and family needs are addressed.
    7. Act as a partner to the family and as a liaison with community providers, school representatives, legal system, and support systems.
    8. Utilize Motivational Interviewing techniques to ensure the cross-system plan of care maintains a focus on strengths, needs and capacities of the child and family.
    9. Accurately complete documentation per required timeframes and recorded in the Electronic Health Record.
    10. Update the cross-system plan of care after each monthly meeting in a strength-based manner.
    11. Provide required reporting for alleged abuse or neglect, parental relapse with drugs or alcohol, safety plan not followed, new safety issues identified, severe behavioral incidents, running away, or situation requiring medical attention or hospitalization.
    12. Lead monthly CFT (Child and Family Team) meetings.
    13. Participate in weekly meetings with the families to help them problem solve and develop an effect time management plan.
    14. Provide families with time to prepare for court hearings, FTDM (family team decision meetings) and other dependency related appointments. Attend appointments to support families and advocate for the family's needs.

    Other Relevant Duties

    1. Carry out shift responsibilities (as assigned) during work hours designated at Rising Strong; staff other program areas as needed to provide support to the program.
    2. Actively participates in staff meetings, multi-disciplinary meetings with other team members and participate in supervision.
    3. When subpoenaed or otherwise called upon, appear in court to testify regarding families' involvement in the program in a professional manner.
    4. As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect
    5. Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI). Per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and 42 CRF, Pt 2.
    6. Will regularly utilize all forms of communication (email, text, phone, etc.) and database practice necessary for this position and as directed by your supervisor.
    7. Maintain a state credential and re-apply annually in order to complete caseload billing requirements.
    8. Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine.
    9. Performs related functions necessary to support the mission and core values of Catholic Charities.

    Job Qualifications

    To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.

    Education/Experience: to perform this job successfully, an individual must have a bachelor's degree in social services, psychology, or education field and a minimum of two years of related experience and/or training or an equivalent combination of education and experience. Experience working with behavioral health services is desired.

    • Basic computer proficiency

    Preferred Skills:

    • Knowledge of Spokane City and Spokane County community resources.
    • Experience working with issues of homelessness, substance abuse/addiction, mental health and/or poverty preferred.
    • At least two years' successful experience in providing case management services and direct client services.
    • Demonstrated experience working with culturally diverse populations and/or demonstrated life experience which qualitatively improves service delivery to Rising Strong families (i.e., diverse health perspectives, cultural and family style approaches).

    Certificates/Licenses: To perform this job successfully, an individual must:

    • Possess valid WA State driver's license and clean driving abstract.
    • Successfully pass background check applicable to position.
    • Eligible for Agency-Affiliated Counselor registration required.

    Workplace Environmental Conditions

    While performing the essential duties/responsibilities of this job, the employee will be:

    • Noise Conditions: exposed during a shift to constant or intermittent sounds at a level sufficient to cause hearing loss or fatigue.
    • Exposure to Trauma: Employees of Catholic Charities Eastern Washington provide direct services to vulnerable community members including those who have behavioral health challenges and those who have been exposed to trauma.
    • Client Population may include persons involved in gangs, with substance abuse concerns, homelessness and/or tendencies towards violent behavior.
    • Potential for exposure to illicit drugs and air-borne contaminants (proper PPE provided).
