    Job Summary and Qualifications

    Responsible for planning and delivering quality home health care services, ensuring compliance with standards and best practices.

    What you will do in this role:

    • Supervise and monitor assigned home health care team(s)
    • Review patient documentation for compliance and accuracy
    • Participate in quality improvement activities
    • Coordinate care and ensure efficiency of clinical associates
    • Educate and mentor home health care team members

    What qualifications you will need:

    • Minimum two years of experience in a licensed home health agency
    • Knowledge of Federal and Local standards
    • Supervisory experience is preferred
    • Active Registered Nurse (RN) License
    • Valid driver's license and reliable transportation
