    The Clinical Team Manager oversees and coordinates the day-to-day operations of the patient's clinical interdisciplinary team, to ensure quality and appropriateness of care and availability of services with appropriate staffing. They serve as a liaison, effectively problem solving and following through on patient care issues, including notification of appropriate personnel, and/or physician. The Clinical Team Manager also collaborates with other clinical supervisors and administrative team members to provide appropriate and continual program oversight, supervision, training and mentoring of clinical personnel as directed by the Director of Clinical Operations and have knowledge of business operations.

    Qualifications:

    • Graduate of an approved school of professional nursing or therapy licensed in the state(s) in which practicing.
    • One (1) year experience in home health nursing or therapy, required.
    • Three to five (3-5) years of experience in their respective clinical field, preferred.
    • Acceptance of philosophy and goals of Agency.
    • Ability to exercise initiative and independent judgment.
    • Ability to collaborate with individuals to enlist cooperation of team to perform/achieve a common goal
