Description

As a Creole speaking Case Manager, you will play a crucial role in providing support and guidance to individuals in need.

Duties:

Conduct assessments to determine clients' needs and develop individualized care plans
Provide crisis management and intervention services as needed
Coordinate with external agencies and resources to ensure clients receive appropriate services
Monitor and evaluate clients' progress towards their goals
Offer guidance on accessing services such as Section 8, Child welfare, and Behavioral health
Utilize motivational interviewing techniques to empower clients
Provide case management services to clients, including conducting assessments and developing service plans
Coordinate and monitor services to ensure clients' needs are met
Advocate for clients and assist with accessing resources and support services
Fluency in Creole and English

Requirements:

Bachelor's degree in Social Work, Psychology, or related field (preferred)
Previous experience in Case Management or related field
Strong supervisory skills to oversee client cases effectively
Knowledge of crisis management strategies and interventions
Familiarity with Medicare regulations and processes

Job Types:
Full-time, Part-time, Contract, Temporary

Pay:
Up to $25.00 per hour

Expected hours:
No less than 20 per week
