Job DescriptionSalary: $31,200-$35,360/yearHealth and Wellness Specialist Job Description
Shift - M-F (8a to 4p)
Responsibilities:
Directly reports to the Clinical Director
Schedule and coordinate structured activities for clients.
Oversees the Health and Wellness plan for all of the assigned facilities
Oversee implementation of daily structured activity schedule
Oversee Operations case management and CPST hours to ensure adequate provision of services
Ensure all structured activities are documented appropriately
Develops the Health and Wellness Assessment for all of the assigned facilities
Collaborate with other managers to coordinate special activities and events
Work with Executive Director to implement program at Mt. Orab Facility
Plan and execute quarterly Family Outreach events
Prepare schedules of activities 3 months in advance and submit to supervision and Human Resources Director for approval
Work with Human Resources Director to acquire necessary funding for activities when needed
Oversee community service programs and activities
Coordinate with community stakeholders/resources to schedule and facilitate community/facility events
Assess and survey client interests and needs regarding activities, then adjust accordingly
Conduct case management and CPST documentation trainings for staff
Lead and instruct recreational activities, classes, workshops, topic groups etc.
Monitor and track services provided
Develop service reports when needed
Develop Caseload, Weekend, Holiday, Weekly and Special activities/programming for facility
Attend all required meetings
Attend trainings, conferences, etc. to continue professional development.
Updates and edits the Health and Wellness Assessment as needed, for all of the assigned facilities
Develops the Health and Wellness Goal Assessment in 3 phases, for all of the assigned facilities

Assess clients in individual sessions and meet with them weekly or every other week for all of the assigned facilities.
Organizes and Leads group fitness, nutrition, other therapeutic activities for all of the assigned
facilities
Plans outdoor and community events for all of the assigned facilities
Minimum qualifications:
High School Diploma
Drivers License
CDCA
The applicant must show proof of holding and must continue to maintain valid permits, licenses and certifications as may be required for this, or demonstrably similar, position as recognized by the Ohio Department of Administrative Services.
The applicant must have a valid Driver's License. Applicant must have the ability to deal with and solve problems involving several variables in somewhat unfamiliar settings and topics.
