Description

Responsible for outreach in the community to providers and stakeholders to increase referrals of children and adolescents eligible for services funded by the Substance Use Block Grant (SUBG).

Complete screening, assessments and Intakes of eligible members. Act as the primary point of contact for all members, activities and services funded by the SUBG.

Facilitate meetings and follow up on recommendations, to ensure continuity of care and service delivery, and educate members about service options and delivery.

Must be able to obtain and retain a Level 1 Fingerprint Clearance Card.


Minimum Qualifications:


  • Bachelor's or master's degree, no experience necessary.
  • Associate's degree plus one (1) year behavioral health work experience.
  • High School Diploma or GED plus Peer Support Specialist certification plus two (2) years of behavioral health work experience.
  • High School Diploma or GED plus Medical Assistant certification plus two (2) years of behavioral health work experience.
  • High School Diploma or GED plus three (3) years of behavioral health work experience.

PLUS:


  • 1 year experience working with persons with substance use disorders,
  • Must be able to obtain and maintain Juvenile Corrections clearance (PCJCC)
  • Must have or be eligible for DPS Level 1 fingerprint clearance and able to maintain clearance throughout employment.

Additional Preferred Education/Experience Qualifications:


  • Bachelors or master's degree in the Behavioral Health (BH)* related field.
  • Experience working with Medication Assisted Treatment programs
  • Preferably Bilingual (English/Spanish)

Important Notes:


Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver's license with a "clean" or "acceptable" driving record.

An unacceptable driving record includes (but is not limited to) the following:

- more than two [2] moving violations in the last three [3] years or;
- more than one [1] at-fault accident within the last three [3] years or;
- any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years.

Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.

Applicants must be able to pass a pre-employment drug screen.
