Our non-profit organization exists to help youth not only live but to thrive.

In response to the pervasiveness of youth homelessness, we specialize in providing services to homeless who are Seriously Emotionally Disturbed and Severely and Persistently Mentally III (SED/SPMI) who may have co-occurring substance abuse.

To fulfill our mission, it is our practice to serve any youth in need.

We offer programs and services such as emergency supportive housing, peer support groups, linkage to health care and intensive case management, and drop-in center services.


Duties and Responsibilities:


  • Oversee all services and operational activities.
  • Provide comprehensive case management that includes, but is not limited to: screening and assessing clients; creating, implementing, and monitoring individual service plans; crisis intervention; connecting clients to health, mental health, employment, housing, and other community resources.
  • Foster a professional and welcoming environment in all interactions with clients, guests, vendors, and colleagues, regardless of differing identities, backgrounds, beliefs, and choices.
  • Manage and evaluate the performance of assigned staff.
  • Develop and maintain professional and supportive relationships with staff and subordinates.
  • Provide information regarding services and transportation as needed to people seeking services.
  • Work in tandem with case managers to assist clients in reaching their case plan goals.
  • Continually monitor and evaluate each client's progression through their case management plan, and develop corrective action revisions to the plan as needed.
  • Maintain accurate documentation of service objectives and outcomes as well as other services by established guidelines.
  • Actively participate in client
- and program-related meetings and training.

  • Adhere to strict boundaries and professional ethics in the care of others.
  • Ensure that the facility is clean, free of potential hazards, and safe at all times.
  • Enforce house rules and policies. Qualifications and Requirements.
  • 4year degree in human services, or a comparable combination of education/workrelated experience in a social service setting; working knowledge of case management and planning techniques.
  • Experience working with homeless individuals, youth (age 1625), and the continuum of care in a similar environment to the population of clients we serve.
  • Detailedoriented and highly organized.
  • Provide own transportation for workrelated travel as necessary.
  • Maintain a clean driving record and valid driver's license. Skills/Ability.
  • Leadership and management.
  • Make sound decisions and has experience handling highly sensitive and confidential information.
  • Multitask in a fastpaced work environment.
  • Proficient in Microsoft Word and Excel.
  • Excellent written and verbal communication skills. Attributes.
  • Genuine interest in helping youth.
  • Patient, inquisitive, resourceful, and flexible.
  • Relatable to the young people we serve. Physical Ability.
  • Must be able to reach, bend, walk, stand and sit for long periods and climb stairs.
  • Must be able to lift a minimum of 30 lbs.

Education:


  • High school or equivalent. (preferred)

License/Certification:


  • Driver's License. (preferred)
