Position Summary


The Espero Rutland Clinical Case Manager is responsible for screening, assessment, and therapeutic services of individuals who suffer from substance abuse.

Espero Rutland is a permanent supportive housing services to people that have experienced homelessness, with a goal of stabilizing them in their housing situation at Espero Rutland, a high-quality apartment community which is deeply affordable and intensely supportive for 171 individuals experiencing homelessness.


Essential Duties & Responsibilities (inclusive but not exhaustive):

  • Provide individual counseling for chemical dependent persons and complete required documentation.
  • Conduct intake assessment, biopsychosocial assessments, individual counseling sessions according to evidence informed goaloriented approach.
  • Facilitate recovery groups.
  • Attend weekly interdisciplinary treatment team conferences.
  • Complete and maintain documentation in a timely and accurate manner and according to Federal, State guidelines.
  • Monitor drug screening.
  • Liaison with external psychiatric care providers to help schedule treatment sessions.
  • Assists in compiling statistics for program reports.
  • Consistently interact with clients using person centered and Strength's based approach while maintaining professional boundaries.
  • Collaborate with community partners to identify families and/or individuals who are experiencing homelessness (or atrisk of homelessness).
  • Ensure that clients are safe and linked to appropriate resources following any crises.
  • Provide crisis intervention, as needed.
  • Build relationships with other service providers that serve the target populations.
  • Be present and active in communitywide activities, meetings, and events.
  • Manage case data by entering it in a timely manner into Service Point, the database utilized by Caritas of Austin for the Homeless Management Information System (HMIS).
  • Regularly prepare and maintain accurate records and reports, following the Caritas of Austin's Housing Services Department Documentation Guidelines.
  • Work with supervisor to create and implement a professional development plan.
  • Treats clients, coworkers, and the general public with courtesy and respect.

Qualifications:


Education
-
Required:Bachelor's degree from an accredited college or university.
-
Required: Licensed Chemical Dependency Counselor (LCDC).
-
Preferred: Master's Degree from an accredited college or university in the area of human services. Includes, social work, psychology, public health and counseling.


Experience
-
Required: Minimum one year of professional related experience
-
Preferred: Past work in housing programs or with people experiencing homelessness; experience working with people with mental health disorders; experience with Crisis Intervention.


Computer Skills

  • Experience with internet, MS Office, and databases required. HMIS (Homeless Management Information System) experience preferred.

Language Skills

  • Bilingual proficiency (English and Spanish or other language) preferred.

Other Requirement

  • Must possess or be able to obtain a valid Texas driver's license within 30 days of employment.

Salary/Benefits:


  • Base Salary Range: $48,762 $58,82
