    Description

    Job Title: Case Manager at BRC

    Company: BRC - Building a more caring and compassionate city

    Description: If you want to have a positive impact on your community, this job is for you. As a Case Manager at BRC, you will be responsible for providing comprehensive case management services to identified chronic homeless individuals. Your duties include meeting clients, ensuring complete and up-to-date clinical files, assisting clients in overcoming housing barriers, preparing clients for housing interviews, and participating in case conferences. You will work full-time, 37.5 hours per week, Tuesday to Saturday from 12pm to 8:30pm.

    Qualifications:

    • High School Diploma/GED required; bachelor's degree preferred, masters ideal.
    • Bilingual Spanish strongly preferred.
    • Excellent communication and writing skills, and computer proficiency required.
    • Experience working with homeless populations and knowledge of HRA and NY/NY application systems.
    • CPR training certification or willingness to take training class in CPR.

    *Vaccination preferred but not required
