- Partner with local community organizations to recruit amazing CHWs
- Lead a team of 5-7 CHWs serving hundreds of clients per year
- Support and coach CHWs so they do their best work for clients
- Analyze reports to ensure your CHW colleagues are delivering excellent care
- Integrate CHWs with care teams working in clinics and hospitals
- Collaborate with community organizations to provide resources for clients
- Leadership experience with a non-profit or community-based organization
- Extensive experience as a CHW, social worker or case manager
- Excellent understanding of the healthcare and social service systems including processes for enrolling in public benefits
- Knowledge of the resources and community-based organizations in Prince George's County/Baltimore
- Comfort analyzing reports and using data to drive performance results
- Excellent oral and written communication skills
- College degree; MSW or MPH a plus.
- Passion for fighting health inequality

Here at IMPaCT Care, we get out of bed every day to change lives, with the most effective intervention in the world for addressing health inequity. We find, train and enable Community Health Workers (CHWs): caring people who keep their communities healthy. CHWs are natural helpers who share life experiences with the people they serve and provide structured advocacy, social support, and navigation to help their clients reach their health goals. We have built the largest and most scientifically-proven CHW platform in the country, used across 20 states by 50 organizations like Kaiser Permanente, the Veterans Affairs Administration and United Way. Across multiple randomized controlled trials IMPaCT has achieved outcomes frequently believed to be out of reach, including $2,500 annual savings per patient, improved mental health and a Net Promoter Score of 94.
