Summary:
Responsible for the coordination of the hospice services program.
Maintains excellent relationships with all primary constituencies—patients and families, employees,hospice members, and the communities we serve. Coordinates Interdisciplinary Group meetings.
Supervises all hospice nurses and home health aides directly or through another staff member. Reportsto the Director.
Essential Duties and Responsibilities
Performs those tasks and/or functions outlined herein or others as directed by the Director.
Implements and makes recommendations for changes to policies annually.
Acts as the Quality Improvement Program monitor, conducts studies and recommends changes.
Assists in the development of the annual budget.
Implements, interprets and makes recommendations for changes to goals and objectives.
Implements, interprets and makes recommendations for changes in work methods.
Assists in the recruitment of new hires; interviews new RN, LPN, Home Health Aides, potential hires and recommends whether to hire; recommends hiring or termination of nursing and home health aidestaff.
Reviews complaints and grievances and makes oral/written reports to the Director.Coordinates and monitors the continuity and quality of care for patients/families for home carecomponent of the program.
Supervises and coordinates pertinent professional contracted employees (either volunteer or paid.)
Maintains lines of communications between the interdisciplinary group members and
patients/families, physicians, other health care providers and other community agencies.
Maintains 24 hour, seven days a week availability of RN care for patients/families.
Assures that nursing documentation accurately reflects both the care delivered and patient condition and participates in the development of the plan of care for patients/families.
Monitors and evaluates the implementation of the plan of care. Collaborates with other health careprofessionals when required.Supervises and schedules care conferences with the patients and/or families and the interdisciplinarygroup.
Assures that patient assessments are accurate, complete and timely.
Acts as a resource and provides education, information and/or training for patients/families andinterdisciplinary group members for specific care or procedure in home care.
Education and Experience:
Registered Nurse licensed in our state is required. Baccalaureate Degree in Nursing required
Minimum of three to five years of professional experience in delivering hospice nursing services, which includes at least two years in a management position with a proven track record.
