Overview:

Purpose Statement/Position Summary:

The Supervisor, Community Health Services will oversee the day-to-day activities of a team of community health workers focused on Enhanced Care Management.

This role involves providing direct support in managing patient challenges, collaborating with management to build the team, and ensuring the delivery of high-quality patient care.

The incumbent will work closely with the Manager to coordinate personnel administration, provide administrative support to the administrative leadership, and assist in leading staff meetings.

Additionally, the Supervisor will oversee Community Health Worker activity, attend leadership meetings, and demonstrate a commitment to a patient and family-centered model of care.


Key Responsibilities:

  • Supervise the daytoday activities of a team of community health workers focused on Enhanced Care Management.
  • Provide direct support in managing patient challenges and facilitating access to resources and referrals.
  • Collaborate with management to build and develop the team, ensuring alignment with organizational goals and objectives.
  • Coordinate personnel administration, including hiring, training, and performance management.
  • Provide administrative support to the administrative leadership, including scheduling, documentation, and reporting.
  • Assist in leading staff meetings, providing guidance and support to team members.
  • Attend leadership meetings to provide input and contribute to decisionmaking processes.
  • Demonstrate a commitment to a patient and familycentered model of care, promoting a supportive and inclusive environment for patients and their families.

Minimum Qualifications/Work Experience: 3+ years of experience in administrative and/or community-based experience in a medical/clinic setting or community-based organization, including at least 1 year of experience working directly with patients and/or clients.

  • Supervisory/leadership experience preferred.
  • Deep knowledge of how to manage patient challenges, access resources, and make referrals.
  • Strong empathy, compassion, and management abilities.
  • Excellent communication and interpersonal skills.

Education/Licensure/Certification:

Bachelor's degree in Public Health, Health Administration, Child Development, Social Work, or similar discipline preferred or equivalent experience preferred.
