    Summary



    This position is located at Gallup Indian Medical Center(GIMC), Diabetes Program, Gallup, NM. The purpose of the position is to provide nursing expertise and leadership in assessing, planning, implementing, and evaluating care provided to individuals with or at risk for Type 2 Diabetes Mellitus. Provides diabetes training and consultation to service unit, tribal, area and IHS-wide; community-based diabetes intervention and prevention on case management and diabetes care.



    Duties


    This position is open until filled.

    • Manage care of clients diagnosed with or at risk for Type 2 Diabetes Mellitus; utilizing professional judgment and the IHS Diabetes Standards of Care
    • Teaches the pathophysiology of diabetes, diet/exercise, medications, prevention and management of acute and chronic complications, self-care and management of diabetes
    • Provides individual and group education Evaluates the psychosocial and cultural aspects of diabetes as a chronic disease
    • Participates in the diabetes program with multidisciplinary team to assess and provide culturally relevant care for patients living with diabetes.
    • Performs clinical procedures when necessary
    • Coordinates diabetes care and patient visits with other departments for the prevention of diabetes-related complications Works closely with other members of the Diabetes Team.
