- Manage care of clients diagnosed with or at risk for Type 2 Diabetes Mellitus; utilizing professional judgment and the IHS Diabetes Standards of Care
- Teaches the pathophysiology of diabetes, diet/exercise, medications, prevention and management of acute and chronic complications, self-care and management of diabetes
- Provides individual and group education Evaluates the psychosocial and cultural aspects of diabetes as a chronic disease
- Participates in the diabetes program with multidisciplinary team to assess and provide culturally relevant care for patients living with diabetes.
- Performs clinical procedures when necessary
- Coordinates diabetes care and patient visits with other departments for the prevention of diabetes-related complications Works closely with other members of the Diabetes Team. Provides diabetes training and consultation to service unit, tribal, area and IHS-wide; community-based diabetes intervention and prevention on case management and diabetes care.Duties
