Your tasks

Outpatient palliative care for patients within the care concept "SAPV in Augsburg and throughout the administrative district of Swabia"
Participation in medical on-call duty throughout the service area
Work in a multidisciplinary team to determine the specialized palliative care needs of patients, their families, and caregivers
Consultation and guidance for outpatient cooperation partners as well as contact with the attending pediatrician or specialist for pediatric and adolescent medicine and specialized physicians
Participation in regular team meetings and close collaboration with nursing staff and psychosocial workers
