  • As the Outreach Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:
  • Incorporate the Trans-Theoretical Model of Behavior Change, as applicable into all aspects of supervision
  • Devise corrective action plans with employees when appropriate and in consultation with the agency leadership team
  • Remain current with industry standards for Drug User Health Services including efforts to reduce disease spread, overdose prevention strategies, and other risks issues associated with target population
  • Utilize data and community reports to identify areas and "hot spots" to direct street based outreach efforts, as well as community engagement efforts including tabling events, etc.
  • Ensure the appropriate disposal of all used syringes as per Agency and Program Policies and Procedures. Ensure continued disposal of regulated medical waste according to state and federal regulations
  • Manually and/or electronically document activities and client/member information and interventions in client tracking systems, software programs, and other program systems Participate in agency's 24/7 Health Hub Hotline on call rotation
  • Outreach Coordinator Minimum Salary: $58,500
  • Outreach Coordinator Maximum Salary: $58,500
  • Outreach Coordinator Schedule: Monday-Friday, 8:304:30, oncall shifts with the hotline rotation
  • Qualifications:
  • Associate's degree in a related field
  • Experience working with a diverse population 13 years of experience
