    Position Overview

    Join our team as a Care Coordinator and make a meaningful impact by providing comprehensive care coordination to enhance the delivery of our care management program. In this role, you will play a crucial part in supporting the well-being of our plan enrollees by recommending healthy lifestyle choices and effectively mitigating the short- and long-term effects of chronic illnesses. Your dedication to optimizing patient care and promoting wellness will contribute significantly to improving the quality of life for our members.

    Job Responsibilities:

    • Ensure the responsible delivery of comprehensive services to enrollees, advocating effectively to resolve any issues they may encounter.
    • Actively participate in multi-disciplinary team meetings to collaboratively assess and plan coordination efforts, ensuring the provision of quality care.
    • Provide education and resources to beneficiaries, families, and providers, empowering them with the knowledge needed to make informed decisions about their health.
    • Proactively engage in quality management program activities, contributing directly to the achievement and ongoing maintenance of program accreditations.
    • Assist in the achievement of contractual obligations, including the cost-effective delivery of services and adherence to budgetary requirements, under the guidance of the Lead, Supervisor, or Manager.
    • Accurately report work hours and expenses incurred during the course of position duties in a timely manner.

    Required Qualifications, Skills, Knowledge and Experience

    • Active, unrestricted current state licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), or a Master's of Social Work (MSW) from an accredited university.
    • RNs must have a minimum of 2 years of pediatric experience, while LPNs require a minimum of 4 years of pediatric experience. MSWs need a minimum of 1 year of related professional experience.
    • Knowledge and familiarity with public sector health coverage systems, such as Medicare and Medicaid.
    • Understanding of the principles, ethics, and precepts of care management service delivery.
    • Proficiency in computer skills, including care management applications, Internet/Web usage, and Microsoft Office (Word, Excel).
    • Strong prioritization and organizational skills, enabling effective management of multiple tasks and responsibilities.
    • Ability to demonstrate purposeful and outcomes-directed critical thinking within daily operational activities.
    • Capacity to receive verbal and written feedback professionally and implement performance and productivity improvements as needed.
    • Skill in interpreting real-time and historical information to inform decision-making and optimize care coordination efforts.

    Compensation

    The compensation for this role is $20.00 to $21.00 per hour. Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
