Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities and ensuring that regulatory compliance and organizational standards are met.
Essential FunctionsProvides targeted case management services as described by the Medicaid manualProvides psychosocial rehabilitative services as described by the Medicaid manualSupports clients in following individualized care plans and identifying case management goalsCompletes documentation on services provided within Valley's standardsMeets productivity expectations as assigned by supervisorDistributes medications as necessaryObserves and collects client urine samples as necessaryTransports clients to appointments as necessary BenefitsMedical, dental (including orthodontics), and vision insuranceHealth Savings Account with a company matchFamily Building benefits401(k) with a 5% company matchInsurance plans including life insurance, short and long-term disability, accident, critical illness, auto and home, and pet insuranceTime off including 15 days of accrued PTO (accrual increases by one day for each year of service), 10 paid holidays, and 2 wellness days10 days of Paid Parental Leave for all new parents for birth or adoptionVolunteer paid timeEmployee Assistance Program and discounted mental health therapyEducation Assistance including tuition, new license, specialized training, and conference reimbursement, student loan assistance, continuing education reimbursement, and paid training time.
