- BS or BA in social work or in a related behavioral science field such as psychology sociology, education, family studies, etc., with a minimum of one year full time experience working with individuals in a human service setting after completing the BA or BS or be a certified alcohol and drug counselor. A Master's degree in social work or related behavioral health field may be substituted for the one year of experience.
- Must successfully complete case management training within 6 months of hire and annual continuing education requirements.
- Organizational skills and basic computer skills.
- Must have a current driver's license and driver's insurance.
- Demonstrates ability to work as an effective member of a multi-disciplined team.
- Demonstrate proven sensitivity to the various cultural and socioeconomic characteristics of Children's Home clients and staff.
- Any youth who is considered ineligible for services is referred back to the referral source with recommendations as to a more appropriate course of action or service.
- To follow the safety guidelines and practices as identified in the program operations manual when providing services and provide a daily itinerary to their supervisor or other assigned person, noting where they will be located at all times throughout the work day.
- Case Manager will notify parent/legal guardian or other referral agencies of all critical incidents within 24 hours.
- Attend supervision by a behavioral health professional who has completed case management training at least twice per month with at least one of these supervisory contacts being on an individual basis and face-to-face.
- Ensure client's needs are fulfilled and follow administrative protocols for request such as vouchers, use of credit cards, training request, etc.
- Attend case consultation and clinical meetings.
- If contact with the client and family does not occur within the required time frames, the case manager is responsible to document the reason in the case record and will continue to make effort to contact the family via telephone and letter, all of which will be documented in the record.
- Development of a specific care plan with the client with periodic revisions with specify the goals and actions to address the medical, social, educational, or other services needed.
- Documentation must adhere to contract requirements following state regulations for case records, case notes, and contain the achievement and progress on goals.
- Discharge plans must be completed on all clients who have had three or more service contacts.
- Documentation must follow time frames as stated in the program operations manual.
- Complete additional releases of information forms with the client, family, and/or legal guardian, in order to have the ability to gather information needed in developing the care plan.
- A comprehensive assessment and periodic reassessment must be conducted according to the program operations manual to determine the client's need for medical, social, educational, or other services.
- Assist the client and families with organizing, documenting and scheduling appointments in areas identified in the treatment plan.
- Provide referral or related activities to help client obtain needed services with medical providers, social providers, educational providers, or other programs and services.
- Coordinate home visits, discharge planning, and provide monitoring or follow up activities necessary to ensure the client's care plan is implemented and needs are being met .
- To have contacts with non-recipients who are directly related that have useful input regarding the client's past, or current functioning, symptoms, adherence to treatment, or other information relevant to the client's behavioral health condition and needs for services.
- Assist with the medical needs of the client, securing guardian approval if needed for medication changes and additions, including phone calls and follow up letters as needed.
- Ensure enrollment of client in an educational program, attend school meetings and Parent/Teacher conferences and complete documentation accordingly.
- Ensure client has crisis numbers for case manager, mental health center, psychiatrist or other support that may be needed and information to call 911.
- Attends meetings and court hearings regarding client's assigned to caseload.
- Provide transportation for clients to various appointments, court, meetings and home visits, as needed.
- Position may require services to be rendered in the home of the client at times.
- Knowledgeable of effective coping skills, crisis intervention strategies, motivational interviewing, stages of change, and community resources.
- Must have the knowledge and awareness of suicide prevention and have the ability to screen for suicide risk.
- Obtain case management training within 6 months of hire and maintain continuing education requirements pursuant to 908 KAR 2:260.
- To maintain professional boundaries with client, family members, legal guardians, community resources, referral sources, and community partners at all time.
- Attend required trainings as assigned.
- Responsibility to be aware of, and follow, state law and respective professional codes of ethics to notify the individual identified and law enforcement officials when a client has made actual threat of physical violence against an identified victim, or an actual threat of some specific violent act, or made a statement of intent to harm themselves.
- Responsibility to be aware of, and follow, state reporting laws for dependency, neglect, abuse, or human trafficking.
- Must be able to perform the essential functions of this position with or without reasonable accommodation.
- Other duties as assigned by their supervisor.

Minimum Position Qualifications:
Position Summary:
The Targeted Case Manager is responsible to assist clients and families in accessing behavioral health treatment, medical interventions, educational, social, development, and other support services. Targeted Case Manager advocates for the needs of the clients and families and works closely with our Outpatient clinical team and community partners to ensure the client's needs are being met across settings. Targeted Case Managers focus on linking clients to local resources and enhancing social and emotional functioning and life skills.
