    Description

    We are looking for a Licensed Practical Nurse in the Jamestown & the southern Chautauqua County area. The schedule is Monday - Friday 8:00 am to 4:30 pm with a possibility of weekend overtime.

    Salary ranges from $50,000 to $56,000 per year based on full-time employment and maximum productivity.

    The Licensed Practical Nurse (LPN) in Home Health plays a key role in delivering quality patient care and providing technical skilled services in patients' homes.

    • Deliver services within the defined practice scope as per state regulations and in coordination with the healthcare team
    • Educate the patient's family on care procedures and maintaining a healthy environment
    • Monitor and report patient responses and condition changes daily to the relevant healthcare professionals

    License Requirements:

    • Active LPN license in the state of practice
    • Valid Driver's License, vehicle insurance, and reliable transportation
    • CPR Certification is mandatory
