Responsibilities:
As a Registered Nurse Case Manager (RNCM), you will have the following responsibilities:
- Provide professional nursing care, complete admission, initial comprehensive, and ongoing assessments pf patients.
- Initiate, develop and update individual appropriate plan of care according to an individual patient's needs.
- Working collaboratively with the hospice physician and Interdisciplinary Team to carry out appropriate interventions.
- Managing and supervising the combined efforts of home health aides, social workers, and chaplains.
- Educate patients and caregivers as appropriate.
- Upholding high standards of compliance regarding documentation and patient care requirements.
- Helping the agency to continuously increase the quality of care.

Qualifications:
- Current New Jersey Registered Nurse (RN) License
- Minimum one year of Registered Nurse (RN) experience
- Willing to travel to the assigned areas
- Must have a valid drivers' license with reliable transportation and liability insurance
- Strong communication and interpersonal skills

Pay:
$80, $100,000.00 per year
