    Bilingual Referral Coordinator - Kansas City, United States - Swope Health Services

    Description

    As the Referral Coordinator for Swope Health Services, you will be a part of a team of caregivers that provide care to the people who need it the most. The Referral Coordinator processes and tracks referrals to outside PCMH department The PCMH emphasizes a team-based approach to health care delivery. This is accomplished by delivering proactive, planned and coordinated evidence-based care. The Referral Coordinator will accomplish this by their primary function of processing and tracking of referrals to outside ancillary services.

    Primary Functions:

    • Ensure the provider has completed all of the necessary information needed to process the referral. Including any forms the specialist may require to be sent in with the referral.
    • Enter all of the necessary information to complete the referral including a) Insurance, B) Authorization Code (if the referral requires prior authorization from the insurance company),
    • Call the Provider/Specialty where the patient is being referred to:
      a. Confirm they are able to accept this patient based on referral & insurance
      b.Let them know you are sending a referral and obtain the name of the person you spoke with.
      c.Ensure the specialist calls the patient with their appointment and instructions
      d.Send all of the referral information.
      e.Also ask the Specialist to please make sure they send a notification if the appointment is not kept.
      f.Also to automatically send all documentation/reports from the visit.
    • Contact the patient to inform them of the referral including: name, address and telephone number and other specific instructions regarding the appointment. Give instructions regarding rescheduled appointments.
    • Document the following in the referral section in eCW: Appointment Date and Time if known; under the Notes Section Tab: Date/Time/Person you spoke with at the Specialist Office, Date/Time Referral was sent; Date/Time when you spoke with the patient to advise them of the referral & appointment time. You must document if the specialist is going to call the patient personally to set up the appointment. If any calls come in from the Specialist office or from the patient you must add additional notes in this section.
    • The Referral will be marked Pending - after all of the above has been completed and documented.
      The Referral Coordinator will then track the referrals on the schedule defined by Swope Health Policies and procedures. :
      All interactions with patient or referring entity need to be documented in the Notes section of the the Referral.
    • Referral coordinator to acquire documentation from specialist/consultant through the use of HIE( Health information exchange portals) or through faxes.
    • The documentation will then be sent to the referring provider to review the documents.
    Successful Candidate Will Have :
    • High school Diploma or equivalent required.
    • Graduation from a Medical Assistant program preferred.
    • Experience working in a medical office, 1 year preferred.
    • Basic Life Support (BLS), certified preferred.
    • Excellent communication skills, customer service oriented, telephone etiquette and exhibit the ability to work with people (staff, visitors and clients).
