    Monday - Friday


    Purpose Statement/Position Summary:

    The Community Health Worker, under the supervision of a Master's level administrator, provides specific services to patients and their families.

    Coordinates case management and social care services. Demonstrates commitment to the principles of family-centered care.

    Minimum Qualifications/Work Experience:
    Experience with community resource navigation, social-care support, and/or health education. Experience with public/government benefits through work or lived experiences, preferred. Bilingual skills, preferred

    Education/Licensure/Certification:
    High School Diploma/GED or equivalent combination of related education and/or work experience.

    Pay Scale Information

    $42,016.00-$72,478.00

    CHLA values the contribution each Team Member brings to our organization.

    Final determination of a successful candidate's starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry.

    The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description.

    Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
