Plans, organizes and implements care coordination services that will address the Social Determinants of Health to prevent re-admission and improve the current health status of patients through linkages to community services and resources.
Evening and weekend hours required.Essential Functions Conducts psychosocial assessments of patients and family members to obtain information about home environment, family relationships, perception of patients illness and coping resources in order to provide emotional support and care coordination assistance.
Encourages understanding and acceptance of any illness as well as the treatment plan. Gathers pertinent information from medical records, hospital personnel and significant others to formulate a comprehensive plan of care. Collaborates with patients/significant others and/or physicians on care coordination needs and makes arrangements for needed services/resources as needed. Documents all interactions and services provided in the clients case file and manages files according to regulatory requirements. Participates in program planning and evaluation activities.Is also accountable for monitoring, documenting, and tracking program and client data and assuring compliance with grant-related or hospital-related documentation requests made by supervisor.
Performs other activities as needed and percentages will shift as program evolves. Participates in staff and committee meetings as required. Enhances professional growth and development through participation in education programs, in-service meetings, workshops and continuing education seminars.Qualifications:
Bachelors degree in social services or related field. Prior experience in care coordination/case management preferred. Bicultural and bilingual English/Spanish preferred. Excellent interpersonal skills.
Requires excellent observation skills, analytical thinking, problem-solving and ability to communicate and interact with all levels of professionals, in addition to good verbal and written communications skills.
Good critical thinking skills:
ability to assess, evaluate and teach. Flexible with a working knowledge of all areas of medicine. Ability to adjust priorities quickly, organize multiple tasks simultaneously.
Self-motivated and able to work independently and as a team member Ability to work flexible schedule to address program needs, including both weekend and evening hours.
