- Develop relationships with insurance partners and work closely on different quality projects.
- Coordinate with office managers on workflows aimed at driving Annual Wellness Visit (AWV) compliance rates.
- Review quality metrics for different programs and work with office staff and providers to close care gaps.
- Work with analytics team meet the reporting needs.
- Must be able to work on time sensitive projects and meet deadlines.
- Ability to prioritize between competing projects.
- Produce error free reports and operation materials to offices.
- Carries out responsibilities in accordance to Richmond University Health Network policies and standards.
- Must have demonstrated strong interpersonal skills and be able to collaborate and work effectively with a variety of individuals and skill levels.
- Associate degree; equivalent experience may be substituted.
- Minimum 2 year work experience preferably in a healthcare environment.
- Experience working in a project based environment.
- Experience with Microsoft Excel and office suite

The Population Health Project Coordinator will drive initiatives designed to enhance quality patient care, meet regulatory requirements, achieve recognition from oversight bodies and maximize incentives. Position will require candidate to utilize EMR to document goals and compliance. Must engage physicians and staff in process and workflow changes designed to meet quality metrics. Position will provide all reports in support of project initiatives.Key Responsibilities:
