The Dane County Human Services Department, Aging & Disability Resource Center (ADRC) Division, has 2 Full-Time(1.0FTE) vacancies for a Care Coordinator Project position in Madison, WI.

DEFINITION
The Independent Living Support Pilot (ILSP) Program is located at the Aging and Disability Resource Center (ADRC).

Under supervision of the Independent Living Support Project Supervisor, this position administers the Independent Living Support Pilot grant program to consumers.

Responsible for identifying, assisting, enrolling into the program. Assists consumers to develop a care plan, find service providers, and connect with fiscal agent. Provides ongoing service coordination, transition consumer off the program when finished and complete disenrollment paperwork.

Has required reporting and data collection expectations due to the nature of it being a pilot with the State of Wisconsin.


EXAMPLES OF DUTIES


Provides customer-driven prompt, accurate and friendly customer service responses to requests for information about an array of topics and for access to the ILSP Program.

Searches through the Information and Assistance resource database, including other written and computer-based information resources to identify, evaluate, and suggest potential programs and services.

Collects customer information and assesses client/family needs in order to provide over the phone, in the office, or at home that best meets the needs/demands of the customer's schedule.

Provides follow up to determine outcomes and provide additional assistance in locating resources and arranging services. Develops and implements customer-driven service plans.

Assists in the identification of individualized, cost-effective, outcome-oriented service options that reflect goals that are personally meaningful and attainable by customers and address immediate as well as longer term needs.

Administers the ILSP Functional Screen to determine functional eligibility. Reviews financial data to ensure financial eligibility for pilot program. Connects customer with fiscal agent. Maintains contact with customer to ensure service delivery. Transitions customers from the pilot program into community programs and connects them with community resources. Completes disenrollment paperwork.


Education, Experience & Special Requirements:


Education, Training and Experience:


Bachelor's degree from an accredited college or university with a focus on human/social services, nursing, education or other related fields.

Must have one (1) year experience in an aging or disability service area (e.g.

, experience in:

case management, residential care, assisted living, adult day program, supported employment, and other areas where people who are aging or have disabilities receive services). Other preferred experience may be in human/social services, nursing, education, or other related fields.


Special Requirements:


Must have and maintain a valid driver's license, and access to reliable personal transportation, or equivalent, while in this position.

Must pass a Criminal and Caregiver Background Check. Availability to work occasional nights, weekends, and during special events.
