- Conduct comprehensive psychosocial assessments of patients to identify their needs, strengths, and challenges.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that address patients' psychosocial concerns and support their medical treatment.
- Provide counseling, emotional support, and crisis intervention to patients and their families facing medical, emotional, or social difficulties.
- Assist patients and families in accessing community resources, such as financial assistance, housing, transportation, and support groups, to address their social and practical needs.
- Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Maintain accurate and confidential documentation of patient assessments, interventions, and progress in accordance with professional standards and regulatory requirements.
- Stay informed about social and healthcare policies, regulations, and resources to effectively support patients and advocate for necessary changes or improvements in service delivery.
- Master's degree in Social Work from an accredited program.
- Current state licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent.
- Strong knowledge of social work principles, theories, and interventions in a healthcare setting.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively with a team.
- Compassion and sensitivity to the needs of patients and families.
- Proficiency in using electronic medical records (EMR) and other relevant software applications.
- Bilingual / Spanish fluency is strongly preferred.
- Ability to travel within the designated service area is expected.
