Jewish Family & Children's Services of Southern Arizona (JFCS) is a nonprofit, non-sectarian social services organization that provides specialized counseling and community services to the greater Tucson community.

***Reporting to the Preferred Communities Intensive Case Management Supervisor, (PC/ICM) Case Manager provides intensive case management services and gap services to refugee and other immigrant clients enrolled in the PC/ICM Program and receiving gag services. JFCS' PC/ICM Program assists clients with particular vulnerabilities and social adjustment challenges (such as serious physical health issues, mental health challenges, developmental disabilities, social isolation, experiences of gender-based violence and more). This position plays a critical role in working with families to ensure a safe transition to stability, promote self-sufficiency, and assist with goal attainment.


ESSENTIAL RESPONSIBILITIES

  • Provides comprehensive case management services to clients by researching community partners and making referrals to appropriate social service agencies, communitybased organizations and/or social service providers.
  • Recruits clients through internal and external referrals
  • Completes eligibility screenings enrollments for all clients recruited by or referred to the PC/ICM Program and gap services.
  • Provide direct assistance to clients based on program and client goal plans, and providing follow up services as needed
  • Develops goaloriented, clientcentered selfsufficiency plans with each enrolled client, addressing their long
- and short-term needs and goals.

  • Provides extended cultural orientation to clients as needed.
  • Guides clients through referrals, providing follow up, checkins and assistance, ensuring all services are linguistically and culturally appropriate.
  • Maintains regular contact and positive rapport with all PC/ICM clients.
  • Work closely with JFCS colleagues to ensure that refugees are integrated into the broader array of services available in the organization
  • Completes case notes and required assessments in online database in a timely and consistent manner.
  • Maintains comprehensive case files for each enrolled client, ensuring client's confidentiality and protection of personally identifiable information.
  • Acquires comprehensive knowledge and thorough understanding of programmatic requirements of PC/ICM Program.
  • Prepares PC/ICM Program and gap services reports as required.
  • Maintain positive relationships with community partners relevant to program activities
  • Submit regular reports to track client outcomes, fulfill reporting requirements, and capture client success stories
  • Document all contacts and services in case files, as appropriate
  • Participate in network and community meetings related to PC/ICM program
  • Assist in emergencies as they arise
  • Other duties as assigned

WORK HOURS, SCHEDULE & FLEXIBILITY
There will be evening and weekends that will require your attendance in order to best represent the Agency. Some flexibility in scheduling is required.


REQUIREMENTS

  • Associate's Degree preferred, preferably in Social Work or a related field
  • Prior professional experience in social work, counseling, community organizing, human services or related specialty preferred.
  • Minimum two years' experience working with refugees or immigrant population
  • Experience working with vulnerable populations preferred
  • Proven ability to work under pressure with deadlines and manage multiple priorities in a fastpaced environment.
  • Strong motivational and teaching skills
  • Demonstrated ability to communicate effectively and diplomatically in a multicultural setting and with limitedEnglish proficiency speakers. Ability to communicate effectively through a thirdparty interpreter.
  • International work or study experience and second language abilities a plus.
  • Valid driver's license, Level 1 fingerprint card, proof of COVID19 vaccine and reliable access to an insured vehicle.
