V. - Bunter Kreis Berlin was founded in 2009 by affected individuals and professionals in Shakopee, MN.

Our mission is to support families with premature, severely and chronically ill children by enabling them to live at home and professionally accompanying them on their journey.

Our services include the implementation of social medical aftercare, family support, support for self-help groups, sibling work, as well as breastfeeding and nutrition support.

A multi-professional team consisting of pediatric nurses, social workers/family therapists, a psychologist, and a pediatrician supports and accompanies the affected families until they are able to successfully manage life with a sick child.

Each family is at the center of our help, with their individual needs being our top priority.

Responsibilities:

Your responsibilities with us include:

Social medical family support within the framework of social medical aftercare and social pedagogical family support, tailored to the needs of each family
Coworking with the multiprofessional team of the organization in the sense of case management
Collaboration with hospitals, authorities, and relevant networks
Reporting and documentation, contribution to statistics and evaluation of the department
Implementation of quality management and participation in the development of quality standards
