POSITION SUMMARY
The LPN Care Coordinator is accountable for assisting the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN in the management and leadership of a specific nursing unit.
This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care.
They also assist in the direction and implementation of the nursing process.This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection; assist with the development of care plan, and implementation of care plan and revision of plan.
The Care Coordinator, LPN coordinates the management of the care of residents through a well-defined care delivery systemESSENTIAL RESPONSIBILITIES AND DUTIES
RESPONSIBILITY:
To assist in the development of the Comprehensive Care Planning Process under the Direction of the RN
Systematically collects data about residents through interview of resident and family, observation, discussion with physician and review of medical records.
Assists and is knowledgeable in the development of the MDS, RAPS and care plan in collaboration with the interdisciplinary team.
Assists in the implementation of the care plan by monitoring responsibility, developing communication systems and following up on outcomes.Attends quarterly and annual care conferences under the direction of the RN, meeting the needs of the resident and family.
Monitors for changes of resident's conditions, reports the changes to an RN, and revises the plan of care as delegated by an RN to reflect the current resident status consistent with facility protocols, and physicians' orders.
Assist in the data gathering for completion of assessments: Tardive Dyskenesia, restraint, falls, pain, bladder, Braden scale, smoking, etc. as delegated by the RNAssists in the data collection for the completion of the functional care plans by other nursing personnel.
RESPONSIBILITY:
Has Knowledge of and Consistently Applies Established Nursing Policies and Procedures According to State and Federal Regulations and Nurse Practice Act
Is knowledgeable of and abides by the residents' Bill of Rights.
Uses standards of care and specific policies to perform nursing care procedures for residents.
Allows and encourages autonomous decision making to resident whenever possible.
Abides by and promotes the Continuous Quality Improvement Program of the Rehabilitation Center including audits as assigned.
Documents resident incidents and reports the incidents to an RN.
Attends in-service and reviews literature to maintain skills in gerontological nursing.
RESPONSIBILITY:
Provides Nursing Cares and Leadership
Articulates expectations and follows up on assignments given to subordinates and assures completion of duties, as directed by an RN or supervisor.
Monitors the performance of other LPN's and nursing assistants and provides direction to ensure that staff performs safely and competently.
Prioritizes and updates work assignments consistent with the number and levels of staff available that shift and acuity of residents care necessary.
Takes the necessary steps to replace appropriate nursing staff to maintain proper staffing levels. This may include working as the staff nurse.Works designated weekends as directed by DON.
Knows and can implement fire, tornado and disaster plans.
RESPONSIBILITY:
Has Knowledge and Consistently Follows Standards of Documentation and Written Communication
Completes charting in accordance with health information standards and facility policy, particularly for residents on Medicare Part A and those with behaviors.
Gathers and documents data on admissions and discharges.
Is knowledgeable in the procedure to facilitate all physician orders and completes procedures to assure accurate follow-up.
Assists with the initiation, reporting and documentation of nursing orders.
Provides report to CNA's and other staff, verbally or written about daily resident care needs and/or significant changes in condition.
Follows up on recommendations of the Dietician.Administers medications accurately according to policy, including handwashing and 5 rights.
Complies with standard of remaining 95% free of medication errors.
Signs out medications and treatments including PRN's according to policy.
Reviews and audits medication/treatment sheets monthly.
Uses proper procedure for label changes.
Follows proper emergency kit procedures.
Follows control substance policy.
FACILITY COMPLIANCE

Provides Rehabilitation Center with current license and keeps updated
Attends outside education/in-services pertinent to your department and designated by ADON.
Attends nurses' meetings of the Rehabilitation Center.
Attends all staff education of the Rehabilitation Center relating to safety, infection control and disease process.
Attends educational programs on Continuous Quality Improvement, customer satisfaction and other such programs, and participates in inspections and audits as designated.
Maintains current knowledge of Minnesota Department of Health regulations.Maintains and applies knowledge of current policies and procedures as outlined by Monarch Healthcare Management.
QUALIFICATIONS
Education and Experience
Must have a current LPN registration in the state of Minnesota.
Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider.
Must possess the ability to communicate verbally and in writing when coordinating the care of the residents.
Must be able to perform essential functions of the position with or without reasonable accommodation.
Must qualify for employment, after criminal background study, per guidelines of Minnesota Department of Human Services.
Preferred
Experience in healthcare, skilled nursing facility or assisted living community
Knowledgeable about Medicare coverage and eligibility
