Position Overview


The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to independently or as a member of a professional team, travel to conduct surveys, investigations and monitoring visits of long-term care facilities and hospitals to determine compliance with State Licensure requirements and/or Federal Medicare/Medicaid certification regulations.

This individual will also serve as technical expert and independently perform difficult and complex institutional health and safety surveys and will also serve as preceptor for lower-level surveyors.

Job Responsibilities


  • Surveys long term care facilities.
  • Travels to conduct difficult, highly complex and sensitive onsite health and safety surveys to determine the level of compliance with state and federal requirements.
  • Appears and testifies at Department and nonDepartment legal proceedings and administrative hearings to defend portions of the team decision related to his/her findings.
  • Interviews facility residents, staff, administrators, health care providers and other individuals.
  • Collaborates with other surveyors on the survey team.
  • Prepares reports of filed surveys and evaluates plans of correction relating to health and safety aspects of longterm care facilities.
  • Serves as preceptor to new hires and existing institutional health and safety survey staff regarding training and orientation.
  • Assists the Regional Supervisor in the assessment and development of new survey staff by identifying training needs.
  • Recommends training programs.
  • Assists with the arrangement for field training.
  • As a member of the survey team, travels to perform difficult and complex health and safety surveys to assure compliance with applicable rules and regulations at long term care facilities and hospitals.
  • Provides technical support on issues related to compliance with regulations associated with environmental health sanitation.
  • Documents findings.
  • Prepares reports of field surveys.
  • Discusses noncompliance relating to health and safety aspects of these facilities with Supervisors and team members.
  • Reviews and evaluates survey and investigation reports of lower-level surveyors and lower-level sanitarians for technical accuracy and compliance with applicable directives, policies and guidelines.
  • Explains and interprets state and federal laws, department rules, regulations, policies and procedures, related to institutional health and safety to providers and interested parties.
  • Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
Minimum Qualifications

  • Requires successful completion of both state and federal Basic Surveyor Orientation courses.
  • Requires certification of having successfully completed the federal Surveyor Minimum Qualifications Training.
  • Requires three years' experience as a health facilities surveyor.
  • Requires completion of four years of college with a minimum of 30 semester hours in the physical and/or biological sciences, supplemented by three years of professional experience in environmental health sanitation or in a health care facility.
Conditions of Employment

  • Requires ability to travel.
  • Requires a valid and current driver's license.
  • Requires ability to meet all agency vaccine/healthrelated polices and guidance.

Work Hours:
Monday - Thursday 7:30AM-5:30/5:00PM (1/2 hr lunch)
